The Annual Church Statistical Report is required by the Annual Conference and General Conference to ensure current contact information is on file and that CMC churches are participating as necessary to remain in good standing.
Just as Churches are required to submit an updated form annually, licensed and ordained ministers are required to submit an annual faith and activity report to the Annual Conference and General Conference to receive their new updated credentials.
